General Release of Personal Image and Likeness
I grant LetsLose, Advanced Weight Loss Clinics Inc., licensees and successors the right to use my name, image, likeness, vocal recording or personal testimony whether written or audio for all forms of media and use in the ordinary course of business LetsLose, Advanced Weight Loss Clinics Inc. participates in. I also waive any right of inspection or approval prior to use. I give this release freely and without compensation for use throughout the world and in perpetuity.
Release
I release LetsLose, Advanced Weight Loss Clinics Inc. and Company’s assigns, licensees and successors from any claims that may arise regarding the use of my image including any claims of defamation, invasion of privacy, or infringement of moral rights, rights of publicity or copyright. Company is permitted, although not obligated, to include my name as a credit in connection with the image.
I have read and understood this agreement and I am over the age of 18. This Agreement expresses the complete understanding of the parties.
